Abstract
A 61-year-old Taiwanese woman presented with septic shock of unknown cause. Portal venous gas formation was noted on abdominal sonography, and was later confirmed by computed tomography. Small intestinal infarction was demonstrated on emergency abdominal exploration. Since air in the portal vein usually signifies intra-abdominal catastrophe, bedside sonographic examination can provide prompt diagnosis that may lead to better prognosis.
