Abstract

According to the statistics provided by the Department of Health, cancer is the leading cause of death in Taiwan. Hepatic cancer ranks first among the causes of death in men while it ranks second among that in women. However, oral metastasis of hepatic cancer is extremely rare. Due to the low number of reported case, the epidemiology, treatment and prevention of this clinical predicament remains poorly understood. This case report presents a 45-year-old male, who was diagnosed with hepatocellular carcinoma in August 2007. He underwent right hepatic lobectomy as well as transcatheter arterial embolization for treatment of residual tumor. In December 2007, the patient had his left maxillary third molar extracted and came to our department after experiencing post-operative pain. Intraoral examination revealed a mass, measured about 2.8×2.5×2.0 cm^3, at the upper left posterior area that bled easily when provoked. A biopsy of the lesion confirmed gingival metastasis of the hepatic tumor. As a result, the patient received radiotherapy to the metastatic site. However, the patient expired due to lung metastasis complicated with malignant pleural effusion eight months after he was diagnosed with gingival metastasis of the cancer.
