The study project will be conducted for 3-year periods with two phases. The purposes and methods of the two phases are indicated as followings: The First Phase: The aims of the first year are to explore the prevalence and related factors for sarcopenia among elderly with diabetes. A cross-sectional design will be adopted, targeting 1100 community- dwelling adults aged >= 65 years with diabetics in three teaching hospitals in Taipei City. We will screen for sarcopenia based on the Asia Working Group's diagnostic method. Data will be collected by means of physiological measurements, questionnaires, and blood biochemical tests. Physiological measurements include usual gait speed, grip strength, and body composition analysis. The questionnaire included background information, barriers related to diabetes, perceived self-management for diabetics, social support, health hazard behaviors, eating habits, and physical activities. One-way ANOVA, point-biserial correlation, and logistic regression analysis will be applied for inferential statistical analyses. The Second Phase: A double-blind experimental study design will be employed to evaluate the effects of the exercise and protein supplementation interventions for improving muscle mass, muscle strength, physical functional for among elderly with diabetes and sarcopenia in the second and third years of this project. Those who diagnosed with diabetics and sarcopenia during the first year will be randomly assign to the experimental and control groups. There are 5 experimental groups including exercise group, high protein supplement group, high protein diet group, exercise combined with high protein supplement group, and exercise combined high protein diet group. The experimental groups will receive 12 weeks of intervention and the control group receive a one-time course after post-test. Each group is consist of 21 elders. Before and after the intervention, the pre-test and post-test will be performed. The repeated measures will used to analyze data.
