Description
Traditional pharmacy education is centered around the medications rather than the patients. Even nowadays with the development of clinical pharmacy, pharmacists are trained with professional knowledge that leads them to tend to manage diseases and clinical outcomes, instead of deepening their understanding of patient experience and provide good patient-centered care. Therefore, the aims of the three-year study are to: (1) Explore the medication experience of patients with chronic diseases in Taiwan (Phase 1), (2) Design a tailored pharmacist intervention to address the medication experience in patients with chronic disease (Phase 2), (3) Evaluate the effectiveness of a tailored pharmacist intervention to address the medication experience in patients with chronic disease (Phase 3). The results of this study are expected to help health care professionals and pharmacists to better understand the personal medication experiences of patients, and apply the intervention developed in the study to establish a method for pharmacists to embrace shared decision making, and improve patient medication adherence and outcomes.
